I just used Google docs. You can go to “Create” and then pick “Form.” It’ll take you to a fairly intuitive pop-up that’ll let you create a questionnaire. Once you’re done making the questionnaire (or save it) you’ll be able to see a spreadsheet that lets you look at the data once you start getting responses.
There’s a bunch of other things you can do (e.g., export the data into an Excel file, stop and restart responses, change the questionnaire items, etc.), so let me know if you have any more questions. I don’t know everything, but I’m somewhat comfortable with it.
