I can’t find any data that shows earlier usage. I would love to know that the term is older than 2004, but most pre-2000 data is lost to the internet ether. as such, 2004 is the first recorded instance of the term that I can find.

Update:
Because we cannot find a true origin of the phrase, all we can say is that it’s earliest known record was Halo 2 in 2004, with Modern Warfare being the catapult that launched it into the mainstream. Thank you for your support and ideas.

The article you linked to in your original post was a dead link, the first two forum posts you provided were also post-MW. The third one, however, does show pre-MW use of the term. We are looking more in depth at the origin of the term. Be patient please.
