With this I'm assuming that any person with an account can decide to turn off (or on) the NSFW (and possibly Spoiler) features?
It was explained by the coder in the past, but basically won't work. In order to do that, you'd have to have a different site cache for literally every site account to understand how they have personally selected what is turned on and off.
It's how the site is coded etc. No idea why exactly, because it works differently on image boorus, but can't have it all.
However if you're thinking something along the lines similar to the "Show Fewer Ponies" button, I guess that could be possible; I would have to ask the coder about it.

Although it has kinda lost its purpose with ponies hardly being spammed as much as in the past.
Would it be possible to just rework the "show fewer ponies" button into "show less NSFW?" You wouldn't need to do much beyond change keywords, and "Show Fewer Ponies" is just about pointless now anyway.
