Here's the last round. I'm surprised how much Henson company specific requests I got, and whittling them down was the toughest part. But with all three categories, I decided to go with one familiar face and one more obscure, lost or one time use character. So representing:

The Muppets:

Rowlf the Dog (from "The Muppets (2011)" during the "Smells Like Teen Spirit" number)
Tug the Monster (From Little Muppet Monsters. Side note: It's a shame they haven't used those Penguin puppets since that show)

Sesame Street:

Captain Vegetable (Guess the hidden reference)
Mr. Snuffleupagus (with Gus, his puppet)

The Jim Henson Company

Chamberlain Skeskis (from The Dark Crystal, technically not a Muppet, but I'm counting it)
Begoony (From Fraggle Rock, specifically the episode "The Incredible Shrinking Mokey")
