From what I understand there's at least 3 possibilities
1. Rose basically did a 're-format' on her gem when Steven was born, so it's just him in there
2. Rose is still in there in some way, shape or form
3. Rose regenerates back after he gets poofed

All 3 could be equally tear-jerking…
