Well, there are two things; Most of the artist's work is pornographic in nature, so it stands to reason this one could be a far more subtle picture of that nature.

Also the picture is improperly titled. Following the source, the title of the picture is Sister. I don't know if that refers to the Prim or the Brion, or both.
