Japan has weird, often lopsided censorship laws. Showing little boy penis as two semi-circles is ok, and so was showing underage nude girls in Stardust Crusaders, but violence is heavily censored outside of blood, among other things. Lets not forget how Jotaro turned into Shadow Jotaro when he used a cigarette (he's technically too young to legally smoke).
