The trouble often is when Representation conflicts with literary Conservation of Detail, especially when it comes to things of a sexual nature. Representation is good, but if it isn't blended into the story well, it can break immersion, and then people may start to wonder about the intentions of the writer. And it doesn't even have to be big and in your face to be inclusive, as long as it makes sense in the context of the scene and story.
I can imagine some instances where this is the marketing department's fault instead of the writers, though. Not the inclusion, but them putting a jarring spotlight on it, leading to the above problem.
