(t Diagnosing With Nurse Redheart It has troubled me for some time seeing that a pony in our town suffers from a chronic eye condition known as strabismus-the failure of the eyes to properly allign in a binocular gaze. However, there are many causes of strabismus so I resolved to pull out my textbooks from my days at Johns Trotkins school of nursing to try and diagnose the condition affecting this fine pony, our friend Derpy Hooves Ms. Hooves clearly suffers from vertical strabismus, wherein the eyes do not allign in the vertical orientation. Futhermore it is inconsistently present, suggesting that it may be dependent on gaze position. Contraction of the superior oblique moves the eye in two different ways d g on the lateral gaze position. The disorder of the superior oblique that seemed like the best fit is a rare disorder called Brown Syndrome Brown Syndrome is a mechanical disorder of the trochlear tendon. In Brown Syndrome, the superior oblique gets stuck in the trochlea and cannot slide like a pulley system, as it usually would This keeps the affected eye stuck in the same vertical position whenever the affected eye points inward, because pointing inward alligns the eye's axis of vertical rotation with the stiff and confined superior oblique muscle. In other words, vision is only derped in some positions. If you look straight ahead, or up and out, then your eyes work fine. But in some directions you get strabismus (derped eyes) I also considered damage to the rochlear nerve- the nerve that controls this same muscle. This could result from physical head trauma damaging only that nerve. A trochlear nerve palsy could also arise from damage to the trochlear nerve's nucleus, located in the pons (brainstem) possibly from a tiny brain tumor, or a tiny intracerebral hemorrhage This doesn't fit as well as Brown Syndrome though For one thing, ponies with trochlear nerve palsies tend to hold their heads downward, seeing the world through an upward-pointed gaze to avoid use of the superior oblique, which is needed to turn the gaze downward. Ms. Hooves does not do this. So there you go rpy, you have It's something a pony is sometimes doesn't mean there's anything wrong with your brain or your intelligence.
