A mock-up of how Mario could have looked when wearing the hat is included. Paper Mario: Sticker Star contains this unused sticker, called "sl_pkn_hat" internally. Based on how these abbreviations are used in other places in the code, this stands for "Seal: Pakkun Hat", meaning "Sticker: Piranha Hat". The game already uses another hat sticker, the Spike Helmet, which protects Mario from aerial attacks when worn. The Piranha Hat was likely either an early or an alternate version of that effect.
