This meme originates from an episode of The Simpsons titled "Brother Can You Spare Two Dimes", which was first broadcast on the Fox Network on August 27, 1992. Here, Homer's half-brother Herb recounts to when he let Homer design a car for his company, which when first unveiled, proved unpopular and drove Herb's company bankrupt. The flashback to the event shows a cover of Forbes magazine with the caption "The Blunder of the Century".
This meme's earliest known uses was in early June 2012 on the Video Game Memes website, when images of the magazine cover were used to criticize the video games announced at that year's E3 Expo, using Photoshop to edit out the car on the cover and replacing it with a logo or picture. It since has been used to show peoples' disgust and negative reactions at a company's upcoming projects.
