Like the title says, have you ever seen something in a move, TV show, etc. and thought to yourself "the internet's gonna meme this isn't it?" and then you wake up the next morning, open up KYM and sure enough, there it is on the front page. I KNOW it's happened to me once or twice (although I can't recall any specifics at the moment).

On the contrary. I've been there to witness an in-joke that never escaped 4chan. Happy Little Bird was its name. I can't even find images of the thread for reference but it was really fucking weird and morbid.
The first was Gak, and the second was the Superbowl Halftime Shark. In both cases it was only a few minutes before they became memes. Get enough people watching the same thing at the same time and have something a little unintentionally odd about it and it happens.

Far as memes go, I witnessed the birth of one that pretty much was related only to a forum that focuses on lol-cows:
When ParkourDude91 was thought to be a legit thing and not some guy who did well in screwing with everyone, his line of saying "Press 1" became a sort of meme similar to that of agreeing with something and sorta like Press F to pay respects.
If it counts: Grape-kun. See him start off as something peculiar yet cute and then it explodes, enough that the VA for the cardboard cutout of some anime waifu he had come in and visit him. And then there is his death which more or less cemented him in case no one really heard of him before.
