Summary
The Creepy Ugly Guy meme is a photoshopped image / edit of 2 stock photos, maybe 3 or 4 from the software Kai's power goo. It was used as a shocker image in videos dating back to the 2000's, and reports of the image have gone back to as early as 1996. The image is also used in the internet video Schfifty-Five.
