BUT
as Justice Frank Murphy said to summarize 1952's Beauharnais v. Illinois:

"There are certain well-defined and limited classes of speech, the prevention, and punishment of which have never been thought to raise a Constitutional problem. These include the lewd and obscene, the profane, the libelous and the insulting or 'fighting' words – those which by their very utterances inflict injury or tend to incite an immediate breach of the peace."

Essentially, yes there is freedom of speech, and yes we should respect everyone right to say their opinion, BUT if someone is advocating violence then it really protected.

It's because it's not very well enforced.
I mean, you have a law that makes it illegal to even accidentally refer to someone with a gender they don't identify with.
I doubt people aren't getting sued because it never happens.
