One of the tires wasn't working right, but they used it anyway, and a chunk of metal exploded out into the stands, which crushed a child's head.

I remember watching some documentary on stunt performers, here and there are shots where the performer literally died doing it (e.g. car jumping a chasm and making an impossible landing), and for various reasons, none of them being due to respect to them and their family, the death shot makes the movie's final cut.
