Unexplained Actor/Element Changes in British Sitcoms

Sometimes I'll be watching a British Sitcom and Suddenly there is a different actor playing as on of the characters, or some other major element will be different. Usually between seasons. And it's never explained, like it was that way the whole time.
For example, in Red Dwarf the Male Supercomputer personality is suddenly swapped with a Female Supercomputer personality.

Is there a name for this Phenomenon?
Am I just crazy?
Has anybody else noticed this happen?

Holly fell in love with his alternate universe self, and they explain he loved her so much he had a sex change.

Any other examples?
