That's Uboa from Yume Nikki. It's a weird face creature you're supposed to have like 1 in 64 chance of encountering at a specific spot in the game by flicking a light switch off. Thing is it doesn't even harm you, it just sends you to a very strange dimension. What's unnerving about it is it's uncanny appearance and the suddenness of its appearance.
