I remember Vivian actually being born male in the Japanese version of the original release, but I cannot recall if Vivian was trans or a femboy. The localization just made Vivian a normal female to maintain the E rating. In fact, the older sisters bully Vivian for being prettier than they are despite Vivian being born male.
