My understanding is the original Japanese was a little confusing on just how explicit the "Vivian is trans" thing is due to some of the grammar, but the remake clears it up with the Japanese script having dialogue that has Vivian outright say "I was born in a boy's body, but I have the heart of a girl" which is a very Japanese way for a trans person to describe themselves.
