In the Octo Expansion, you have to go through a series of tests to get above ground. If you mess up, then a balloon on your back will inflate and explode, killing your character. This is accompanied by a character, known as CQ Cumber saying whatever you did wrong, and then "Test Failed". The reason this is a meme is some of the small things you die for, and how you violently explode whenever it is said. It happens extremely often as well due to the game's challenging levels.
