Old usage
While there do exist Old English spellings of words with "'d" (in fact, being very common in old print for the normal "ed" ending), the practice fell out with contemporary times.
Meme
As used in forums and chatboards, the "'d" addition to nouns have come to be a way to "verbize" a noun. This is generally used to imply a feature of a noun as an action.
A variant of the meme is to add the "'d" to the end of either famous people, movies, or other popular culture items as to imply that the poster of a previous message had suffered a fate or experienced an action similar to that person, movie, etc.
Four examples of using celebrities for this include these from Urban Dictionary for 'd : (Each of these further have definitions at the Urban Dictionary).
-He O.J.'d when he got away with murder.
-He Kobe'd when he raped that girl.
-He R. Kelly'd when he slept with a 13-year old.
-He Michael Jackson'd when he touched that 7-year-old boy.
Origin
There are several possible origins for the adaption, all placing the emergence around 2003-2004:
One is the shortening of the term pwned to "pwn'd", and varients of that. Exactly when the "pwn'd" variant entered popular culture is difficult to assess; a date search in Google shows "pwn'd" coming into common usage by around 2004.
The MTV show Punk'd first aired in 2003 and lasted 5 seasons. The show involved tricking celebrities and others through hidden camera jokes. As a popular show with younger crowds, also at the time of continued growth of the Internet, the mutation of the phrase could easily come from here.
Another traceable origin of "'d" slang is from Homestar Runner. Part of the cartoons of Homestar Runner include segments called "Teen Girl Squad", a cartoon "drawn" by the character of Strongbad. The cartoon features four teenaged girls that suffer from horrible fates imagined by Strongbad. In the first Teen Girl Squad (made available in 2002), one of the characters is killed by a man that shoots arrows out of his mouth, with a phrase of onomatopoeia citing "Arrowed!!!" (yes, the first use lacked the "'d" statement)
The man, later called the Arrow'd Guy, would reappear in future Teen Girl Squad comics, killing one or more of the characters with a similar onomatopoeia but now using the "'d" addition, including:

- Cerebellum'd
- Late 360 Shove-it to Boneless'd
- Maniac in a Speedo'd
- Dangled off a Hotel Balcony'd
- Alternative Energy Source'd
