TL;DR: They supposedly had sandbags in chairs on the plane to simulate passengers, but forgot to secure them so the sandbags all shifted to the back of the plane upon takeoff and shifted the plane's Center-of-Mass behind it's Center-of-Lift, causing it to lose control and fall tail-first into the water and shatter apart.
