The female term for "Don" is "Donna", which is used for the wife or female family members of a mob boss. "Donna" simply means "woman" in Italian, but in this context it's used for a woman of high status, from a wealthy family or that deserves respect
It's also used towards people who don't necessarily are part of mafia. For example, my father calls an old lady "Donna Rosaria" and the owner of his favorite pub "Don Vincenzo".
It's almost exclusively something used by the previous generation of Southern Italians, so I don't think they use this in the North or Center Italy
