If I put enough indirect actors and rely on context that people define as "that's just how things are", then the guy's dead and nobody is responsible and there's nothing anyone can do about it.

Sophistry. There is already a duty to help those in critical condition or in an impoverished state in America. The REAL issue is the price tag that comes AFTER the fact (except if you're impoverished, in which case it's usually a tax write off). Yes, the alternative isn't great, I get it, but to say that there's criminal negligence like some kind of capitalist equivalent to a communist famine and blaming the people who hate violent solutions isn't fair.
