EXPOSITION TIME! (I just made all this up.)

See, at the very beginning of the game, you know how your character's all freaked out and panicky? That tattoo instills him with the knowledge and experience of a seasoned warrior. That's how he can shoot so accurately mere minutes after picking up a gun for the first time. It also teaches him how to slink through the jungle quietly and dress his wounds properly. (And by "properly" I mean "Don't do this shit. It'll hurt like hell and do more harm than good.")
