What this man is suffering from isn't just a chest bursting tadpole that was true polymorphed into a heart and used in a shoddy organ transplant, resulting in the heart's gradual deterioration back into a tadpole which will double kill him but also the curse of a hag coven that causes any attempt to alleviate this condition to turn him back into a red slaad, since he was also true polymorphed. He also has Lyme disease.
