yes, they're familiars that you can get in Binding of Isaac: Rebirth, the problem is that you need to get the four horsemen of the apocalypse to get they're full versions and get the achievements, but when trying to unlock them the traditional way you can get this guy who doesn't drop them and will prevent the achievement.
