The ouch face is a face from the status bar in the doom series it is caused when a player takes great damage and seen rarely since it is considered a glitch in the game the same face later appeared in the xbox version of doom when crossing lava or taking blast damage
STEPS TO DO THIS
1.FIND A LARGE HEALTH LIKE SUPERCHARGE
2 SHOOT A ROCKET INTO THE WALL JUST AS U GET THE HEALTH
