The Bowser in the Fire Sea course contains a group of Goombas. These have a chance of randomly exploding when they walk near the edge of their platforms. Note the Goomba in the bottom left corner of the footage dying for seemingly no reason.

When Mario falls off the Wing Mario Over the Rainbow secret course in Super Mario 64, he will fall into the moat. His health is not reset during this. If Mario falls after hitting a ledge and reaching 0 HP, he will instantly drown upon touching the water.

The Mad Piano only wakes up at the sound of Mario running, not walking or jumping. With a combination of walking and a backwards somersault, it is possible the collect the red coin the piano is guarding without waking it up.

If Mario lands right on the edge of the floating island in Cool, Cool Mountain, he will teleport down to the level's death barrier. As Mario is immune to death while in his "recovering from snow" animation, he will first finish it before instantly dying.

It's possible to be blown off the bridge in Snowman's Land in such a way that Mario's hat lands under the star in the ice structure. This makes the hat irretrievable without restarting the game as Mario will always touch the star before putting it back on
