The Bowser in the Fire Sea course contains a group of Goombas. These have a chance of randomly exploding when they walk near the edge of their platforms. Note the Goomba in the bottom left corner of the footage dying for seemingly no reason.

When Mario falls off the Wing Mario Over the Rainbow secret course in Super Mario 64, he will fall into the moat. His health is not reset during this. If Mario falls after hitting a ledge and reaching 0 HP, he will instantly drown upon touching the water.
