1 in 1000 failure for ballpoint pens isn't that bad, but you should deal with it anyway because it's going to ruin your profit margins and brand trust (if failure is from whole faulty batches instead of spread out) otherwise.

1 in 1000 failure for commercial airplanes is an unprecedented disaster, companies would be ruined and most people wouldn't fly ever again.

Difference here is lives. The building of the Itaipu Dam had, according to more conservative and probably censored numbers (many swear for more) give about 150 dead out of a bit over 40,000 workers (0,00375% fatality rate). That's genocide by today's standards. 1.3% death rate for any non violent incident is fucking bonkers if you ask me.
